This assessment is intended for adults. We do not knowingly collect information from individuals under the age of 18.

Information We Collect

When you complete this assessment, we collect only the information you voluntarily provide:

• Your first and last name
• Your business name
• Your email address
• Your phone number (optional)
• Your self-reported assessment responses and written reflections

No payment information is collected through this assessment. No account creation is required.

How We Use Your Information

Your information is collected solely to:

• Connect your assessment results to your client record at 4D Financial Solutions LLC
• Prepare for your Legacy Strategy Consultation or advisory session
• Follow up with you regarding your results and next steps

Your information will not be used for general marketing purposes without your consent. You may opt out of any follow-up communications at any time by contacting us directly.

How Your Information Is Processed

Your assessment responses are processed locally within your browser. When you submit the assessment, your contact information and results are transmitted securely to 4D Financial Solutions LLC through Formspree, our trusted form processing provider.

Formspree processes form submissions on our behalf and is bound by its own privacy policy. No other third-party services receive your personal information through this assessment.

What We Do Not Do

4D Financial Solutions LLC does not:

• Sell your personal information to any third party
• Share your information with outside organizations not affiliated with your advisory engagement
• Use your information for advertising or remarketing purposes
• Collect payment information through this assessment

Third-Party Services

This assessment uses the following third-party services in a limited capacity:

• Formspree — Processes and delivers form submissions securely to 4D Financial Solutions LLC
• Google Fonts — Loads typography assets for display purposes only. No personal data is shared

If you proceed to schedule a Legacy Strategy Consultation, payment is processed through Stripe. Stripe's privacy policy governs the handling of your payment information.

Data Retention

We retain your information for as long as necessary to fulfill the purpose for which it was collected or as required for ongoing advisory services. You may request deletion of your information at any time by submitting a written request to admin@4dfs.net.

California Privacy Rights

If you are a California resident, you may have additional rights under applicable privacy laws, including the California Consumer Privacy Act (CCPA). 4D Financial Solutions LLC does not sell your personal information. To exercise any privacy rights, please submit your request in writing to admin@4dfs.net.
